ABSTRACT

Background: Third-party disability (TPD) has been studied in multiple patients including those with aphasia and hearing loss. Only one study has been done in relation to caregivers of adults with dysphagia. Third-party disability has been analysed using the International Classification of Function and Disability (ICF) framework. This study, therefore, used the ICF model to explore TPD of caregivers of adults with dysphagia for the context of Johannesburg in South Africa. Objectives: To describe how caregivers experience TPD when caring for adults with a dysphagia in Johannesburg. Methods: Data were collected from five primary adult caregivers, who were all family members, from government clinics in Johannesburg. This article reports the findings from the interviews that were analysed thematically using a top-down analysis approach. Results: Caregivers experienced challenges related to TPD mostly related to difficulties of being able to do activities of daily living for themselves, their household chores and attending social engagements. The use of body structure and function from the ICF model was not overtly applicable to the caregiver population. A new visual representation has been suggested to highlight the key themes to augment the social and psychological changes as seen on the ICF framework and demonstrated the specific interaction that these factors had on one another. Conclusion: Third-party disability is present in caregivers of patients with dysphagia. Healthcare workers need to be aware of the impact that this can have when preparing home management strategies. This newly devised representation can assist in creating a locally relevant patient-centred care approach but requires future input.

ABSTRACT

OBJECTIVE To provide reference for the regulation of electronic prescription circulation under the mode of “medical science + pharmacy”. METHODS Based on evolutionary game theory, a tripartite evolutionary game model was constructed for related parties of electronic prescription circulation (government agencies, internet hospitals and third-party drug platforms) under the mode of “medical science + pharmacy”. The influential factors of the three parties’ strategy selection were analyzed and verified by simulation. RESULTS & CONCLUSIONS The strategic choices of all parties in the game were affected by the other two parties. Reducing the cost of strict supervision or increasing the punishment for the other two parties would increase the probability of strict supervision of government agencies. Enhancing reputation losses, increasing penalties, and reducing the cost difference when choosing different strategies would have regulatory and binding effects on internet hospitals and third-party drug platforms. It is suggested that government agencies can introduce diversified supervision mode and establish an integrated “intelligent supervision + traceability” system; pay attention to the construction of reputation mechanism, and promote the coordinated linkage of all parties; increase the punishment for violations, provide policy support, reduce the cost of compliance behavior to promote the high-quality development of electronic prescription circulation under the mode of “medical science + pharmacy”.

【Objective:】 Third-party research participants recruitment companies and their recruitment businesses are in the ascendant. To analyze the current situation of company construction, clinical trial application, and ethical review, and discuss and propose ethical management suggestions based on work experience. 【Methods:】 The recruitment of research participants related to the projects approved by the ethics committee in our hospital in the past two years was investigated by questionnaire to comprehend and analyze the data of the third-party recruitment company in construction, application, and ethical review. 【Results:】 This paper found the problems of the third-party recruitment company in the application of clinical trials, the company’s own construction, and the standardization of ethical review, and analyzed the phenomenon and causes of the problems based on work experience. 【Conclusion:】 Although the application of third-party recruitment companies is becoming more and more extensive, it is still in a mixed state of pros and cons. It is necessary for all parties to work together to further standardize and constrain them, so as to protect the rights and interests of research participants and effectively support clinical trial research.

ABSTRACT

The assessment of the Party branch is conducive to improving the quality of Party building, giving full play of the role of Party branch, and better realizing the two-way integration of Party building and health care, to promote the high-quality development of public hospitals. Oriented by problems, West China Hospital, Sichuan University comprehensively adopted a series of methods, such as literature research, Delphi method and in-depth interview and so on, to construct the index system of the Party branch assessment and explored an effective operation mechanism. As a result, the basic management was consolidated, the normalization and standardization of Party branch work was advanced, and the roles of Party branch became more prominent, which is expected to provide decision-making and work references for health authorities and national counterparts.

El texto analiza la crisis política que enfrentó Brasil, del 2013 al 2018. Se analiza cómo se vivió la crisis y cómo se articularon en la conciencia de los ciudadanos los diferentes elementos que constituyen esa coyuntura. Se realizó un estudio empírico con 158 universitarios para entender cómo se traduce la lucha ideológica en Brasil en la opinión de las personas sobre los tipos de conflicto político y cómo se reflejan estas representaciones en las percepciones sobre la crisis. Los datos muestran la existencia de una fuerte polarización política y corroboran que la forma en que el individuo se ubica en el espacio político es muy importante para comprender el significado que le dará a la crisis. Finalmente, el estudio muestra la importancia de las representaciones de la estructura social del conflicto como matriz ideológica subyacente al comportamiento político.


This text analyzes the political crisis that Brazil faced, from 2013 to 2018. It analyzes how the crisis was experienced and how different elements that constitute this context were articulated in citizens consciousness. An empirical study was carried out with 158 undergraduate students in order to understand how the ideological struggle in Brazil translates into people's opinion about the types of political conflict and how these representations are reflected in perceptions about the crisis. The data show the existence of a strong political polarization and corroborate that the way in which individuals locate themselves in the political space is very important to understand the meaning that they will give to the crisis. Finally, the study shows the importance of the representations of the social structure of the conflict as an ideological matrix underlying political behavior.


O texto analisa a crise política que o Brasil enfrentou, de 2013 a 2018. Analisa como a crise foi vivida e como os diferentes elementos que compõem essa situação foram articulados na consciência dos cidadãos. Foi realizado um estudo empírico com 158 estudantes universitários para entender como a luta ideológica no Brasil se traduz na opinião das pessoas sobre os tipos de conflito político e como essas representações se refletem nas percepções da crise. Os dados mostram a existência de uma forte polarização política e corroboram que a maneira pela qual o indivíduo está localizado no espaço político é muito importante para entender o significado que dará à crise. Por fim, o estudo mostra a importância das representações da estrutura social do conflito como matriz ideológica subjacente ao comportamento político.

Resumo O que explica a produção legislativa dos ministérios federais no Brasil? O artigo explora a relação entre a instabilidade ministerial e o desempenho dos ministérios mensurado pela quantidade de propostas legislativas produzidas entre 1999 e 2014. Por meio de um modelo de média exponencial com regressores endógenos, argumentamos que a troca de ministros e ministras afeta negativamente a capacidade de produção legislativa das pastas. Ainda assim, o distanciamento ideológico entre os partidos do ministro e o presidente, bem como a expertise legislativa prévia dos ministros, tem impacto mais pronunciado no nível de produção de propostas legislativas dentro da agenda política do governo. Os resultados corroboram a percepção do efeito negativo da instabilidade ministerial na eficiência da concepção e da formulação de políticas públicas, contribuindo para o entendimento da produção legislativa e do desempenho burocrático do presidencialismo brasileiro.


Resumen ¿Qué explica la producción legislativa de los ministerios federales en Brasil? El artículo explora la relación entre la inestabilidad ministerial y el desempeño de los ministerios medido por el número de propuestas legislativas producidas entre 1999 y 2014. Utilizando un modelo de promedio exponencial con regresores endógenos, argumentamos que el cambio de ministros afecta negativamente la capacidad de producción legislativa de los ministerios. Aun así, la distancia ideológica entre los partidos del ministro y del presidente, así como la experiencia legislativa previa de los ministros, tienen un impacto más pronunciado en el nivel de producción de propuestas legislativas en la agenda política del gobierno. Los resultados corroboran la percepción del efecto negativo de la inestabilidad ministerial sobre la eficiencia de la concepción y formulación de políticas públicas, además de contribuir a la comprensión de la producción legislativa y desempeño burocrático del presidencialismo brasileño


Abstract What explains the legislative production of Brazilian ministries? The article explores the relationship between ministerial instability and the performance of ministries measured by the number of legislative proposals they produced between 1999 and 2014. Using an exponential average model with endogenous regressors, we argue that the change of ministers negatively affects the entities' legislative production. The ideological distance between the minister's and president's parties and the prior legislative expertise of the ministers, have a pronounced impact on the level of legislative production on the government's political agenda. The results corroborate the perception of the negative effect of ministerial instability on the efficiency of the conception and formulation of public policies and contribute to the understanding of legislative production and bureaucratic performance of Brazilian presidentialism.

ABSTRACT

Adhering to and strengthening the Party′s overall leadership is the primary guarantee for promoting the high-quality development of public hospitals. How to comprehensively strengthen the internal audit led by the Party and improve the efficiency of internal audit governance in public hospitals, so as to play the role of internal audit in effectively promoting the high-quality development of public hospitals, is an urgent problem to be solved. The author analyzed the current situation and practical reasons of the internal audit under the Party′s control in public hospital in China, constructed the mechanism framework of the internal audit under the Party′s control, and further analyzed and processed the implementation path of the internal audit under the Party′s control from three aspects: establishing the Party Committee Audit Committee to strengthen the top-level design of the internal audit, linking the three major audits to give full play to the supervision force of the internal audit, and implementing the eight major restructuring to achieve the closed-loop governance of the internal audit under the Party′s control.

ABSTRACT

Upholding the overall leadership of the Party is an important cornerstone for promoting development of public hospitals. Party building leadership for high-quality development of public hospitals must adhere to the value orientation of public welfare, establish a modern hospital management system, and promote the mutual promotion and integration of both Party building and medical service. The practice path of Party building in leading the high-quality development of public hospitals in a tertiary hospital won the support of patients, employees and medical students at large, gave full play to the leading role of the Party, and made breakthroughs in terms of medical service, discipline construction, talent training and internal management quality improvements. By strengthening top-level design and institutional system construction, the hospital has effectively transformed the political and organizational advantages led by Party building into an endogenous momentum for the reform and development of public hospitals. The practice solved such important questions as to the beneficiaries of the high-quality development of public hospitals, whom to depend on and how to govern, hence providing reference for further promoting the high-quality development of public hospitals in China and realizing the deep integration of Party building and medical service.

ABSTRACT

People as third-party observers, without direct self-interest, may punish norm violators to maintain social norms. However, third-party judgment and the follow-up punishment might be susceptible to the way we frame (i.e., verbally describe) a norm violation. We conducted a behavioral and a neuroimaging experiment to investigate the above phenomenon, which we call the "third-party framing effect". In these experiments, participants observed an anonymous perpetrator deciding whether to keep her/his economic benefit while exposing a victim to a risk of physical pain (described as "harming others" in one condition and "not helping others" in the other condition), then they had a chance to punish that perpetrator at their own cost. Our results showed that the participants were more willing to execute third-party punishment under the harm frame compared to the help frame, manifesting a framing effect. Self-reported anger toward perpetrators mediated the relationship between empathy toward victims and the framing effect. Meanwhile, activation of the insula mediated the relationship between mid-cingulate cortex activation and the framing effect; the functional connectivity between these regions significantly predicted the size of the framing effect. These findings shed light on the psychological and neural mechanisms of the third-party framing effect.

Este estudo busca analisar os processos de transição que vêm ocorrendo na vida cotidiana da comunidade Kalunga em relação à percepção das danças entre as gerações de idosos e jovens. Foi realizada uma pesquisa etnográfica utilizando entrevistas e observação direta na comunidade. A interpretação ocorreu com base em autores que discutem o tema no campo das ciências sociais no diálogo com a educação física. Como conclusão, podemos afirmar que os corpos se entrelaçam nas festas e danças e promovem diálogos de dança estabelecidos entre gerações. O sistema ritual, constituído por festas e danças, revela os sentidos e significados das danças como uma estratégia de resistência cultural, permitindo a reconstrução do passado e a ressignificação do presente desse grupo social (AU).


This study seeks to analyze the transition processes that have been occurring in the daily life of the Kalunga community in relation to the perception of dances among the elderly and young generations. An ethnographic research was carried out using interviews and direct observation in the community. The interpretation occurred based on authors who discuss the topic in the field of social sciences in the dialogue with physical education. As a conclusion, we can say that the bodies are intertwined in parties and dances and promote dance dialogs established between generations. The ritual system reveals the senses and meanings of the dances as a strategy of cultural resistance, allowing the reconstruction of the past and the re-signification of the present of this social group (AU).


Este estudio busca analizar los procesos de transición que se vienen dando en la vida diaria de la comunidad Kalunga en relación a la percepción de los bailes entre las generaciones mayores y jóvenes. Se realizó una investigación etnográfica mediante entrevistas y observación directa en la comunidad. La interpretación se dio a partir de autores que discuten el tema en el campo de las ciencias sociales en el diálogo con la educación física. Como conclusión, podemos decir que los cuerpos se entrelazan en fiestas y bailes y promueven los diálogos de danza establecidos entre generaciones. El sistema ritual revela los sentidos y significados de las danzas como estrategia de resistencia cultural, permitiendo la reconstrucción del pasado y la resignificación del presente (AU).

Apresentamos e compreendemos a festa do Divino Espírito Santo na Barra do Guaicuí-MG enquanto vivência de lazer e constituição de identidade de jovens ribeirinhos. Discutimos os termos: Lazer, Juventude, Festa e Identidade e apropriamos da etnografia como método, realizando uma pesquisa de campo com observações, questionário e entrevistas. A festa do Divino diz da construção de identidades possibilitadas pela articulação dos fenômenos religiosos e o lazer, por meio do acesso a bens socioculturais tradicionais e participação no contexto contemporâneo. Os jovens imersos nos afazeres religiosos, ao mesmo tempo em que rezam, dançam com os catopés, ouvem músicas sertanejas e forró, postam fotos nas redes sociais, dentre outras experiências. Todo esse cenário e atores constituem uma forma de compreender a cultura e as identidades desses jovens, que misturam tradição e contemporaneidade não em oposição, mas em diálogo.


We present and understand the Divino Espírito Santo's party in Barra do Guaicuí/MG as a leisure experience and identity constitution of young riverside people. We discuss the terms: Leisure, Youth, party and Identity and embrace ethnography as a method, conducting a field research with observations, questionnaires and interviews. The Divino's party says about the construction of identities made possible through the articulation of religious phenomena and leisure, along the access to tradition sociocultural goods and participation in the contemporary context. Young people immersed in religion activities, at the same time that they pray, dance with catopês, listen to sertanejo and forró music, post photos on social networks, among other experiences. This whole scenario and actors are a way of understanding the culture and identities of these young people, who mix tradition and contemporaneity not in opposition, but in dialogue.

To strengthen the Party′s overall leadership over public hospitals is the cornerstone to ensure nonprofit and sustainable development of public hospitals.It is a vital guarantee to promote the Party construction work at public hospitals by establishing and improving the hospital′s internal management organization, management system, rules of procedure and handling system. This study aimed to establish the operation system of the rules of procedure of the Party committee meeting of public hospitals by building an office automation platform. This system could connect functions covering topics determination, meeting convening, resolution implementation, and supervision in an effort to achieve a closed-loop management and to realize the controllability, feedback and traceability of the decision-making process. The closed-loop management could not only ensures that the decision-making process be scientific and standardized, but also the decision-making efficiency, for reference in the Party committees of other public hospitals in improving their efficiency and quality of debate and decision making.

Objective:To analyze the status quo, trend and influencing factors of patient satisfaction in tertiary maternity and child hospitals in China.Methods:Based on the survey results of five round third-party evaluations of the China Healthcare Improvement Initiative from 2016 to 2021, descriptive trend analysis was conducted on the satisfaction index in tertiary maternity and child hospitals, and the influencing factors of satisfaction were analyzed based on the results of the fifth evaluation.Results:Compared with the first round, the overall patient satisfaction in tertiary maternity and child hospitals in the fifth round was improved(inpatient: from 95.7% to 98.0%; outpatient: from 87.8% to 94.9%). Outpatient satisfaction varied significantly among gender, hospitals of different types, education level, types of treatment, medical insurance and doctors′ titles( P<0.05). Outpatient satisfaction in local hospitals was 1.502 times higher than that in hospitals under the National Health Commission. The satisfaction of referrals was 0.259 times lower than that of patients who were admitted directly. Inpatient satisfaction varied significantly among department, hospitals of different regions, household registration type and whether the admission was delayed due to the COVID-19 epidemic( P<0.05). The satisfaction of inpatients in medical ward and surgical ward was respectively 0.202 and 0.298 times lower than that of inpatients in pediatric ward. Inpatient satisfaction at the central regions was 3.311 times higher than that at the eastern regions. Conclusions:The overall patient satisfaction in tertiary maternity and child hospitals in China is improving. In the future, we should pay more attention to the hospital environment, humanistic care and other non-medical service dimensions with low satisfaction, so as to further improve the medical experience of patients in maternity and child hospitals.

Objective:To summarize the progress and achievement of inpatient experience improvement in public hospitals in China from 2016 to 2021, so as to provide basis for further improving hospitalization experience and raising inpatient satisfaction.Methods:Based on the five Third-party Evaluations of China Healthcare Improvement Initiative from 2016 to 2021, descriptive trend analysis was conducted on inpatient process, cost and inpatient satisfaction, and the inpatient satisfaction in different regions and different types of institutions was compared.Results:The overall satisfaction of hospitalized patients in the fifth evaluation in 2021(98.5%)was higher than that in the first evaluation in 2016(96.2%). The western region and maternal and child institutions had the most obvious improvement in the comparison of regional and institutional types. The improvement of inpatients′ satisfaction with diet was not obvious. Inpatients were most dissatisfied with " high medical expenses" , " cumbersome procedures" and " long waiting time for bed" .Conclusions:From 2016 to 2021, the satisfaction of hospitalized patients has been consistently improved, but there is still room for improvement in non-medical services. Hospital meals for inpatient and relief of inpatient healthcare financial burden are the priorities for healthcare improvement and raising hospitalization satisfaction in the next stage.

Objective:To summarize the progress and achievement of outpatient experience improvement in tertiary public hospitals from 2016 to 2021, so as to provide basis for further improving outpatient experience and raising outpatient satisfaction.Methods:Based on 5 round third-party evaluations of the China Healthcare Improvement Initiative from 2016 to 2021, descriptive trend analysis was conducted on the outpatient process and outpatient satisfaction, and stratified analysis was conducted on patient satisfaction in different regions and different types of institutions.Results:One hundred and thirty-six core sample hospitals were included in the 5 round evaluation, and 30 153 valid questionnaires were collected in the fifth evaluation in 2021.From 2016 to 2021, the overall satisfaction of outpatients in the 5 round evaluations was 91.4%, 87.3%, 90.1%, 91.1% and 95.4%, respectively. The satisfaction of outpatients to the medical services continued to improve. Outpatient treatment process has been continuously optimized, appointment registration has become the most commonly used way of outpatient registration. The satisfaction of outpatients to the process and environment of treatment in the western region was significantly improved.Conclusions:From 2016 to 2021, the overall satisfaction of outpatients has been consistently improved. Informatization helps to optimize the treatment process, but it is recommended that hospitals should provide special support for elderly outpatients.

Objective:To explore the change trend of patients′ recognition of the medical reform effect and medical staff′s occupation and its relationship with medical experience, since the implementation of the China Healthcare Improvement Initiative(CHII), so as to provide reference for exploring the breakthrough point of consolidating the reform effect.Methods:Four indicators of the third-party assessment of patients were selected, which were " I think the medical reform has benefited patients" , " I think the doctor-patient relationship is improving in the past three years" , " I think medical personnel deserve social respect and recognition" , and " I am willing to let my children join the medical profession" . Descriptive and trend analysis were carried out on the survey results, and correlation analysis was carried out between the survey results and patients′ medical experience.Results:Compared with the baseline evaluation results(76.9% and 69.9%for outpatients, 87.3% and 85.6%for inpatients), the patients′ recognition of the sense of medical reform and the improvement of doctor-patient relationship in the fifth evaluation(86.2% and 89.1%for outpatients, 91.4% and 92.6%for inpatients)were significantly improved. The proportion of patients who thought the medical staff was worthy of respect and willing to let their children work in the hospital in the fifth evaluation(97.4%, 79.1%for outpatients, 98.7%, 85.1%for inpatients)were higher than the baseline evaluation results(92.3%, 71.0%for outpatients, 98.1%, 81.6%for inpatients). The above indicators were positively correlated with the patient′s medical experience.Conclusions:Since the implementation of CHII, patients′ recognition of the sense of medical reform and the improvement of doctor-patient relationship has been continuously improved, and patients′ respect and recognition of professional identity of medical personnel is at a high level and continues to improve. Improving medical experience has a positive effect on the improvement of patients′ sense of medical reform.

ABSTRACT

Objective:To assess the progress and impact of the China Healthcare Improvement Initiative(CHII) from 2015 to 2020, which aimed at improving patients healthcare experiences.Methods:Five rounds of evaluation surveys were conducted from December 2015 to March 2021 among 136-214 tertiary public hospitals. Structured questionnaires to measure hospital actions and performance, satisfaction of patients and medical staff were designed, and the data were collected by the national and provincial survey teams via onsite inspection, face-to-face/telephone interviews and self-administered cellphone APPs respectively. After data cleaning, the results of five round evaluations were analyzed and compared.Results:The sampled hospitals demonstrated high participation rate in the CHII, and their overall performance turned out to be satisfactory. And the patient healthcare experiences consistently improved over the period of CHII. In contrast to the high satisfaction rate with clinical quality of the sampled hospitals, patients were less satisfied with the non-clinical services such as food and supporting services. The job satisfaction of doctors and nurses experienced mild improvements, while the problems of heavy workload, low salary, and poor health were not effectively solved yet.Conclusions:The implementation of CHII from 2015 to 2020 has played a significant role in promoting patient-centered service models in hospitals, but continuous inputs and coordinated efforts are still required in the future. The improvement of non-clinical services still needs to be addressed to further enhance patient satisfaction. The job satisfaction and well-being of doctors and nurses remain at low level, and effective measures need to be taken to maintain the stability of the medical staff.

ABSTRACT

In the new era, enhanced Party construction of public hospitals is the fundamental assurance for the success of Healthy China strategy. Based on field survey, in-depth interview, literature review and promotion of pilot practices, the Hospital Party Construction Guidance Committee of Guangdong Province staged a " Four Virtues" program to enhance Party construction at public hospitals in the healthcare system of the province. This program, featuring " leadership competency" , " Party branch capability" , " Party construction branding" , and " Unit showcases" , aims at the limited implementation and homogenization problems commonly found in Party construction at public hospitals. Promotion of successful pilot practices of several public hospitals effectively encouraged the " parallel development" of both medical service and Party construction, providing useful references for the hospitals in their reform/development and Party construction in the new era.
